He was giving a 1 hour special (HBO, I think) and he just keeps giving a continuous stream of sometimes obscure references and throw-away lines that are a challenge to keep up with. The episode must have been near the start of the Iraq war, judging by some of the references. No, there wasn't any other context that I remember. IMO he's the most intelligent comedian out there and he has been on Bill O'Reilly every week.

MILLER: Well, people — there are people who theorize that they make their children sick so they can then save their children. And I think some of the people who are really out on the far left as far as the global warming thing like to keep the planet in a perpetual state of triage, because then they are able to rescue it and feel integral to the process.

O'REILLY: OK.

MILLER: I think that might be what's going on here.
